The CHOICE Arrangement in plain English

No jargon. Here’s exactly how it works.

01

A defined monthly allowance

A CHOICE Arrangement is a health reimbursement arrangement that lets an employer give each eligible employee a set monthly dollar amount toward individual health coverage.

02

Tax-free for everyone

Employer contributions are tax-deductible. Employees receive the allowance tax-free. No payroll taxes on either side.

03

You set the budget

Choose different allowance amounts for different employee classes (full-time and part-time, salaried and hourly, by work location, and more).

How it reduces costs

Three structural inefficiencies in traditional group plans, and how a CHOICE Arrangement removes each one.

01

Flat, disclosed administration

Traditional group plans carry substantial administrative fees. A CHOICE Arrangement replaces that with a flat $45 per eligible employee per month Enough! Health administration fee.

02

No pooled renewal

Group plans pool all employees together, so one year's claims set everyone's renewal. With a CHOICE Arrangement your cost is the allowance you set, and it changes only when you change it.

03

Prices shown before care

Every employee gets the Enough! app and the Care Approved Card: posted prices before they go and an advisor one tap away, whichever carrier they chose.

What it costs

One flat administration fee. Your allowance is yours to set.

  • $45 per eligible employee per month.
  • QSEHRA (for employers under 50 with no group plan): $35 per eligible employee per month.
  • No setup fee for groups that come through a broker.
  • Priced per eligible employee, so an employee who opts out does not change your invoice.

Under 50 with no group plan? The QSEHRA may fit. $35 per eligible employee per month.

How we set it up

Design and documents take about three weeks. The notice period is set by law.

  1. Week 1

    Design

From your census we set classes, the allowance table (age and family tiers), scope (premiums only, or premiums plus care) and any §125 plan.

  2. Weeks 2 to 3

    Documents

Plan document, summary plan description, the employee notice built on the CMS model, the annual ERISA-status notice and any §125 document, signed electronically.

  3. Before the plan year

    Notice and enrollment

Employees are noticed at least 90 days before the plan year starts. An arrangement set up less than 120 days before its start date can notice employees by the first day it takes effect. Employees shop during open enrollment or their 60-day special enrollment window with a licensed advisor and confirm their coverage.

  4. Monthly

    Payments

The Care Approved Card pays the premium. You fund only what is spent, from your own account.

  5. Ongoing

    Run and report

Eligibility, life events, the annual opt-out and renotice, Form 1095-C, the PCORI fee and Form 5500 when required, tracked on a calendar built from your plan.

Frequently asked questions

Is a CHOICE Arrangement ACA compliant?+

Yes. The CHOICE Arrangement is an individual coverage HRA authorized under federal law. An applicable large employer that offers an affordable allowance satisfies the employer mandate, and employees use the allowance on ACA-qualifying individual coverage.

What happens to unused allowance?+

Unused allowance stays with you. Carryover within the plan year is a plan-design choice and is off unless you turn it on.

Can I offer different amounts to different employees?+

Yes. A CHOICE Arrangement lets you set different allowance amounts for different classes of employees: full-time and part-time, hourly and salaried, by work location, and more. Within a class, the amount can vary by age and family size.

What if an employee doesn't buy insurance?+

An employee must be enrolled in individual coverage or Medicare to use the allowance. They confirm their coverage once a year before the first payment and once a month after; the card stays off until they do.

How long does implementation take?+

About three weeks of design and documents. Employees must be noticed at least 90 days before the plan year; an arrangement set up less than 120 days before its start can notice them by the first day it takes effect.

Marsh McLennan Agency is an independent licensed insurance agency and may be paid by insurance carriers. Employees offered an affordable CHOICE Arrangement cannot claim premium tax credits; employees offered one that is not affordable may opt out and claim them. Enough!
